Terms of Service

Effective Date: January 12, 2026

1. Acceptance of Terms By accessing this website and booking services with Pro-Form Soccer LLC, you agree to be bound by these Terms of Service and all applicable laws and regulations in the state of Washington. If you are signing on behalf of a minor, you represent that you are the legal parent or guardian.

2. Description of Services Pro-Form Soccer LLC provides indoor soccer training, skill advancement programs (including "Little Kickers" for ages 2-5), and facility rentals. All sessions are conducted at our facility located at 1009 Ault Field Road, Oak Harbor, WA 98277.

3. Assumption of Risk and Liability Release

Acknowledgment of Risk: Participant and/or Guardian acknowledges that soccer is a physical sport involving risks of injury, including but not limited to collisions, falls, and overexertion.

Release of Liability: To the maximum extent permitted by law, you hereby release, waive, and discharge Pro-Form Soccer LLC, its owner Frank Smith, and all employees from any and all liability for personal injury, property damage, or wrongful death arising out of participation in training sessions.

Indemnification: You agree to indemnify and hold harmless Pro-Form Soccer LLC against any and all claims, suits, or actions of any kind for liability, damages, or costs.

4. Medical Authorization & Safety

Emergency Care: In the event of an emergency, you authorize Frank Smith (who maintains active CPR and AED certifications) to administer first aid or seek emergency medical services.

Health Representation: You certify that the participant is in good physical health and has no medical conditions that would prevent safe participation in soccer activities.

5. Payment, Refund, and Financial Aid Policies

Fees: Training sessions are billed at the rate of $60 per hour unless otherwise specified in a membership agreement.

Payment: All bookings must be paid in full through the website prior to the session start time.

Financial Aid: Pro-Form Soccer LLC proudly accepts Island County Youth Financial Aid. Recipients must provide valid documentation from the county prior to registration.

Cancellations: We require 24 hours' notice for cancellations. Failure to provide notice may result in forfeiture of the session fee.

6. Photo and Media Release You grant Pro-Form Soccer LLC an irrevocable, royalty-free license to use photographs and video of the participant for marketing, social media (including @pro.form_soccer on Instagram), and website promotional purposes.
